What actually changes after 35
Two things change: the number of eggs remaining (ovarian reserve) and the proportion of eggs that are chromosomally normal (egg quality). Both decline gradually through the 30s and more steeply after 38–40. Uterine health, hormonal balance and general health remain very good for most women well into their 40s.
Investigations worth doing early
An AMH blood test and antral follicle count give a snapshot of ovarian reserve. A pelvic ultrasound checks for fibroids, endometriosis and structural issues. Semen analysis for a partner is essential and often overlooked. Together these tests take a few days and clarify whether time-focused trying, targeted treatment or fertility preservation makes most sense.
The role of minimally invasive surgery
Fibroids distorting the cavity, hydrosalpinges, endometriomas and pelvic adhesions each reduce fertility measurably. Laparoscopic and hysteroscopic correction of these problems — before IVF where relevant — meaningfully increases pregnancy rates and reduces miscarriage.
Practical priorities after 35
- Seek review after 6 months of trying, not 12
- Have both partners investigated together
- Treat fibroids, endometriosis and polyps early
- Discuss egg or embryo freezing if pregnancy is being delayed
